Description

The Brockenbrough Phenomena, also known as the Brockenbrough-Braunwald-Morrow Sign, is noted on cardiac telemetry in patients with hypertrophic obstructive cardiomyopathy. This sign reflects the paradoxical decrease in arterial pulse pressure with a rise in left ventricular systolic pressure during the presence of a premature contraction such as a PVC. The case was seen in the cardiac ICU from a 76-year-old female with a history of HOCM. She had been experiencing increasing symptoms of dyspnea on exertion, palpitations, and lightheadedness over the past several years that increased in severity. After a recent visit to a cardiologist concerning these symptoms, she decided to seek intervention for her condition through a septal alcohol ablation of the cardiac myocardium. The patient was examined 1 day status post procedure, and the Brockenbrough sign was present. This clinical analysis can be used in HOCM patients before and after interventional procedures to analyze the improvement of the outflow obstruction.
