A new era of healthcare has begun: one based on quality improvement and value-based care, preventative screenings, and digital records and reporting. In this study, an analytical tool known as “Population Manager” was used in conjunction with electronic medical records to examine and increase current screening rates for colorectal cancer, cervical cancer, breast cancer, and pneumococcal vaccinations. This tool was applied to a Federally Qualified Health Center in Albany, Georgia, Albany Area Primary Health Care (AAPHC), in four of eight primary care clinics. After implementing Population Manager, screening rates were found to significantly improve.

During the two-month research period there was significant growth in our test clinics as compared to the control clinics where Population Manager was not used. The breast cancer screening rate was raised by 21% (27% to 48%), the cervical cancer screening rate was raised by 8% (61% to 69%), the colorectal cancer screening rate was raised by 17% (72% to 89%), and the pneumonia vaccination rate was raised by 18% (53% to 71%).

The goal for the Population Management position is to ensure and increase patient compliance with current preventative screening protocols. By employing this position and the population manager software, AAPHC can provide high quality health care by actively monitoring compliant patients while seeking out noncompliant patients. This position strives to improve health outcomes for the AAPHC patient population.
